Worth re-iterating that this will not sync to a Mac itself (the question asks about Kindle, iPad, and iPhone, so alas, I guess this answers that specifically). But if you want to sync between Kindle for Mac and a Kindle, you may be SOL – Brad Solomon Sep 6 '19 at 1:57.

Try resetting the iPhone by holding the Home and Sleep/Wake buttons until you see the Apple logo as described here: http://docs.info.apple.com/article.html?artnum=305743
Also make sure the iPhone is updated to the latest iPhone software available, currently 2.2.1.
If the issue persists, try contacting Kindle Support via phone or checking their support site.
